What You’ll Do:

• Provide skilled nursing care including wound care, medication administration, chronic disease management, and client assessments
• Develop and follow individualized care plans
• Coordinate care with a multidisciplinary team
• Document visits using an easy-to-use tablet and EMR system

What You’ll Need:

• Active RN license in Vermont
• Ability to work independently and travel locally within Washington County and Montpelier County
• Strong communication and time management skills
